Use a permanent marker to write the name of the dish and cooking directions on a 1-gal. resealable freezer bag

Place first 5 ingredients in bag, burp and seal before freezing

To cook, place the frozen ingredients in a slow cooker and pour in the container of chicken broth. Cook on LOW until chicken is thoroughly cooked (165º), about 6 to 8 hours

Serve hot topped with Mexican-style shredded cheese and tortilla chips
